Why throat dampers stop working
The damper most houses have is a cast iron plate at the throat of the firebox, sitting directly in the path of everything going up and everything coming down. Soot, rust and grit build on the seat and the plate warps with heat over decades.
Even working perfectly it is metal on metal. It was designed to close, not to seal. On a cold night you can feel the draft past a shut throat damper.
Repair before replacement
A seized damper is often just fouled. Cleaning the seat and pivot, freeing the mechanism and replacing the handle restores it, and that is the right first step.
Colonial fireboxes
In genuinely old Massachusetts fireboxes the original cast iron damper is frequently cracked, missing, or was never there at all. Rebuilding one in place is masonry work; a top-mount damper avoids opening the firebox entirely.
Top-sealing dampers

Mine is rusted solid. Can it be freed?
Often. Try cleaning and freeing before replacing. A cracked or warped plate is the point to consider a top-mount instead.
Can I leave it open all the time?
Only while a fire or live embers are present. Otherwise it is a permanent opening, and in summer it lets humid air down into the flue.
We have gas logs. Does it matter?
Yes, differently. Vented sets usually require the damper fixed part- open so products cannot spill into the room — which is exactly why so many are found clamped and forgotten.
